Worksheet to Take to your Pediatrician
To help produce lasting improvements in a child’s weight, many parents find it helpful to keep a food diary to monitor exactly what the child is eating. Toward that end, use this worksheet for the next 4 days to record what your child eats, from the time he or she awakens to when he or she goes to sleep.
